Procedure for Notification of Intent
to Enroll in Internships

All graduate students are required to notify the Nursing Program Coordinator of intent to enroll in a nursing internship one year prior to taking that internship.  Intent to enroll is submitted on a special form.  See details below and forms at right.


The procedure is as follows:
  1. One year prior to the first internship: Submit a signed, completed paper copy of the Internship Intent Form for your specialization to Connie Uhlken, Nursing Program Coordinator, in Room 190 at the School of Nursing by the END OF WEEK TWO OF THE SEMESTER ONE YEAR PRIOR TO YOUR FIRST INTERNSHIP.  You must also attach a copy of your signed planned sequence of courses form; you should have developed this plan for taking courses in collaboration with your faculty advisor soon after admission to the program.
  2. If your plans for taking an internship change, notify the Nursing Program Coordinator no later than the end of week two of the semester immediately preceding your planned internship.  You must submit a revised Internship Intent Form.
  3. All nursing graduate students should contact the Preceptor Coordinator, Dr. Diana Mertens, immediately after submitting the Intent Form and obtaining approval from Ms. Uhlken to begin locating preceptors for the internships.
  4. This is the only notification that is required UNLESS YOUR PLANS CHANGE!
  5. For each internship, contact the faculty member assigned to teach the internship by the second Monday in March (for summer or fall internships) or October (for spring internships) to discuss and submit your internship planning packet information and objectives. Approval of those materials is made by the assigned faculty member and necessary prior to your enrollment in each internship.
